Penis Anatomy

Glans

The glans, or head, of the penis is an area with many nerve endings. In uncircumcised males, it is covered by the foreskin when flaccid. The structure is made of spongy erectile tissue. If a patient is dissatisfied with the size or shape of their glans, they may seek soft tissue enhancement with HA fillers.

Shaft

The main body of the penis, the shaft, surrounds the urethra and fills with blood during an erection. Three columns of tissue, the 2 corpora cavernosa, and the corpus spongiosum become rigid as blood vessels become engorged. This area of the penis is targeted with fillers to increase girth.

Dartos Fascia

The dartos, or superficial fascia, is a layer of connective tissue found in the penis, scrotum, and foreskin. Surgeons use the dartos as an access point for injection of the filler. (2)

Buck’s Fascia

Buck’s fascia is deeper connective tissue. It covers the three erectile columns within the shaft. Surgeons inject superficial to the deep fascia to avoid damaging vascular structures. (2)

Procedure

Before the procedure, Dr. Asandra will provide information so you are fully prepared and comfortable. Once you are at our facility, he will reconfirm your treatment plan with you before administering a medication to the corpora cavernosa to induce semi-erectness. This makes the procedure easier to perform. After this, using a blunt cannula for safety, he selects specific points on the shaft to deliver the filler. The plane between the Dartos fascia and Buck’s fascia is the target placement area. (2)

The volume of filler injected depends on patient preference and the treatment plan outlined by Dr. Asandra at consultation. Once multiple areas have been addressed, manual massage helps the filler to spread evenly throughout the penis. Finally, Dr. Asandra will apply a topical antibiotic, then wrap the area in a tubular bandage. 

After around 2 weeks, the filler will settle fully and patients will see their final results. Biocompatible HA fillers like Juvéderm break down naturally over time, and patients can expect their increased girth to last between 12 and 24 months.

How painful are penis fillers?

Every patient is different, but the process is well-tolerated by the vast majority of patients. In most cases, only mild, brief discomfort occurs during the procedure. In the recovery period, patients may experience tenderness that lasts up to a week.
